Recovery Coach/Case Manager

Responsibilities:
The Recovery Coach is responsible for conducting institutional family support team meetings to prepare youth coming from incarceration.
The primary function of this position is to provide a mentoring role, assist in recovery planning, directly link to services, and conduct consistent check-ins with youth and family.
The Recovery Coach will also keep track of records and efforts in NRI database system; create linkage agreements with other service providers; maintain contact with parole department; and conduct outreach on the streets to ensure capacity is reached.

Qualifications:
Must possess a High School diploma or its equivalent;
Good oral and written communication skills;
Willingness to work flexible hours;
Possession of a valid Illinois motor vehicle operator’s license and willingness to use personal vehicle in course of employment.
